If you are a new patient you may make an appointment for an evaluation or you may schedule a brief, free consultation to determine if we will be a good fit.  

If you would like to make an appointment for a full initial evaluation, please use the appropriate evaluation link. Do not use the free consultation link.  You will receive the necessary intake forms and once completed, I will confirm your appointment.  

You may book a brief (it will last no more than ten minutes) telephone or video consultation at no charge. Please use the free consultation link for this. You will receive a screening document to fill out and once it is completed, I will confirm your appointment. IMPORTANT:  There is no doctor-patient relationship implied or assumed during a free consultation and no treatment or advice will be rendered. It is simply a way to meet me first.
